We want to let you know that after October 1, 2024, Crossroads Animal Clinic will no longer be seeing patients. Due to very limited availability of veterinarians, we have made the difficult decision to close the practice. The last day of normal business operations will be October 1, 2024. Following that, you will need to establish care with a new veterinary practice.
If you have a pet on chronic medications, please make sure to get any refills you may need from us before that. We will not be able to refill medications after October 1. If your pet receives periodic injections in the clinic, it will be especially important to establish care with another veterinarian in a timely manner to continue those medications. Clients who use our online pharmacy will not be able to have new prescriptions or additional refills approved after October 1.
Hewitt Veterinary Hospital, will be offering a complimentary wellness exam, subject to availability, to patients seen at Crossroads in the past 12 months so that your pet can establish care with a new veterinarian to receive medications or treatments without an additional exam fee. (See additional information). They will have access to your pet’s records, so you don’t need to forward those to them.

Terms:
Discount does NOT apply to sick patient exams, diagnostics, treatments, or medications.
Patient must NOT already be a current patient at Hewitt Veterinary Hospital.
Patient must have had an exam at Crossroads Animal Clinic within the past 12 months.
Limit one per patient. Clients with multiple pets may receive the discount for each pet.

Patient Records Request
If you need to have a copy of your pet’s records sent to you or another veterinarian, please fill out this form. Hewitt Veterinary Hospital will have access to your pet’s records, so you will not need to send those if you are establishing care at Hewitt Veterinary Hospital. If you would like us to send records to another veterinarian, please include the clinic name, clinic phone, and clinic email. If you do not enter an email address for the receiving veterinarian, we will ONLY send records to you, and you will be responsible for forwarding records to them.
